I am pretty sure it was before 2005.

The thing is there is no statement on the IPS website that addresses
this, and from forums and the like it appears to have had an impact-
GP's apparently stopped some signing of them as trhey didnt want their
passport details on others forms.
Almost any professional can sign the forms now - so you are not stuck
with the old restricted list of GP, vicar, JP ....
